TITLE:      Therapist–Crisis Residence

I.        SUPERVISOR:             Program Administrator – Crisis Residence

II.       HOURS PER WEEK:   40 hours (includes evenings and one weekend day)

III.      QUALIFICATIONS: LMSW, LCSW, LMHC or other relevant license and experience working with individuals, families and groups                                                 preferred. Ability to function as part of a multi-disciplinary treatment team.

IV.       FUNCTIONS

  • To provide clinical respite services to individuals and families through the use of short-term, individual, group, and family                    therapy.
  • To establish readiness of youth for reintegration into the community by collaborating with families, services providers, and                  community resources.

V.        RESPONSIBILITIES:

  • Coordinate the youth’s treatment plan and develop comprehensive clinical assessments on assigned individuals and families.
  • Facilitate clinical groups as assigned throughout week.
  • Maintain regular contact with assigned clients/families, outside service providers, and referral sources as necessary.
  • Provide individual, group and family focused treatment/therapy including outreach and advocacy to all service recipients.
  • Meet at least once a week with treatment team members in rounds to coordinate treatment planning. Meet informally with team members on an as-needed basis.
  • Complete discharge summary and assist with discharge planning on each assigned case.
  • Coordinate and complete necessary reports and correspondence.
  • Communicate with Agency Behavior Support/QI staff regarding support needs, including potential crisis situations, visitation, and any emergency situations.
  • Manage and coordinate critical situations and precipitous discharges.
  • Attend weekly individual and team/group supervision.
  • Assist with case planning for each assigned case/family as needed.
  • Assist in implementing effective intervention strategies to de-escalate behavior.
  • Maintain updated assessments and progress notes in the SAI Electronic Health Record as required by contract or regulation.
  • Attend and/or facilitate treatment meetings between service providers, family, and youth to discuss progress made, readiness, for discharge, and identify services and referrals needed.
  • Attend all Crisis Residence staff meetings and supervisory meetings, as required.
  • Provide transportation in circumstances dependent on youth and/or family needs.
  • Keep abreast of current trends in clinical treatment, trauma-informed care and evidence-based models through reading, in-service training and conferences. Attend all mandatory in-services and outside workshops as required by the agency.
  • Adhere to NASW, NYS Justice Center and SAI Codes of Conduct/Ethics; follow all OMH standards.
  • Perform/complete other duties as required/assigned.
