St. Anne Institute:  SAI is a private, not for profit, secular residential & community-based preventive service agency committed to providing the highest level of care & rehabilitation services in the briefest time to children & families throughout NY.

Description of Duties: SAI is currently seeking a full-time Outpatient Quality Improvement Specialist to assist Community Based Clinical and Care Management Services with record keeping and administrative support. This position will be part of a clinical team that works to help children, families & adults with the goal of preventing harm and fostering healing.

Quality Improvement

  • Assist the Director with developing & maintaining the electronic health record reports
  • Compile statistics for funding source reports, census, and outcome data
  • Secure and maintain program statistics & accurately track data
  • Input and maintain client demographic data in the electronic health record
  • Coordinate case record review, and other quality improvement initiatives

Billing

  • Develop, implement, & maintain a system of communication with the SAI Business Office for billing of contracts, programs, and third-party insurance providers
  • Assist supervisory staff & the SAI Business Office with monthly agency/contract billing
  • Track and collect client co-payments including self-pay or sliding scale fees
  • Coordinate the activities of Private Insurers
  • Provide continuous confirmation and/or facilitation of the insurance enrollment
  • Develop, implement, & maintain a system of communication & notification among Private Insurance companies, the client & referring partners including new enrollment & disenrollment

Administrative Office Duties

  • Secure intake information/referral packets on all incoming referrals
  • Ensure timely response and updates with referral sources
  • Maintain and update transportation lists as needed/required
  • Provide phone coverage for assigned office
  • Provide first response information to clients with questions regarding appointments or requests for contact with therapists and/or case managers
  • Process and track any purchase orders
  • Maintain program/client records including filing of case related materials, organizing, and filing contracts of individual service providers/referring agencies
  • Write/compile staff meeting minutes and maintain and post to staff.

Case Management

  • Maintain regular contact with assigned clients/families, placing agencies, and outside service providers
  • Coordinate services and client advocacy to each assigned case
  • Provide non-clinical and outreach services based on service needs assessment of client caseload, including transportation, home visits, parent skills training, assistance with daily living skills, safety planning, supervised visitation, and additional services as needed
  • Maintain accountability requirements, including progress notes, Family Assessment Service Plans (FASPs), and contact sheets as applicable
  • Work cooperatively with client’s therapist, meeting on a regular basis and functioning as a member of a multidisciplinary team
  • Assist in discharge planning

Qualifications: BA/BS in medical/public administration or related field; AA and 3 years’ relevant work experience may be substituted for educational requirements. Proficient computer skills, including Microsoft Word, Excel and Outlook; Experience using Electronic Health Records; Familiarity with Medicaid systems of care; Strong time management and organizational skills; Excellent written and verbal communication skills; Able to work effectively and collaboratively within a highly diverse multidisciplinary team; Efficient documentation and recordkeeping skills; Well-developed engagement and problem solving skills; Ability to interact with clients and staff in a professional manner.
