Through our innovative Community Connections program, aftercare begins at intake. Community Connections is a prevention and aftercare program for residents from Albany, Rensselaer and Dutchess  counties.  As soon as a youngster is placed in our residential treatment program, a Community Coordinator begins to organize family and community support for her successful re-entry into the community. Community Connections Coordinators strive to reduce the youngster's length of residential treatment by acting as a liaison between St. Anne staff and the student's home school, parents and various community organizations.
